Áfeorsaþ musa is correct - though in Modern English spelling, it would be áfeorsath musa.
Quick question - as there were multiple versions of "to go away" on the translator, did you pick áfeorsian because it had the best fit meaning (while most of the verbs had just "to go away", this one had depart/dispel in there, as well), or because it was at the top of the page?
No harm if it's the latter, but just a heads up to at least glance through all the possible forms to find the right one.
And if it's the former, more props to ya, and keep that up. :D
